The problems she faces are realistic and her constant yoyo-ing between wanting to lose weight and accepting herself as is rings familiar to me as an irl fat woman. Seeing the other characters acknowledge Sana for her abilities and positive qualities, and there's little mention of her needing to lose weight from prominent characters. I am excited to see how the more adult themes get addressed in future chapters!(4)

But... this whole cousin thing gave me a huge ick (yes not siblings but still) so I stopped reading. Like you could introduce a different female character without blood relations - an old childhood friend, someone he shares activities with but no, it had to be a cousin.
